how would you go about adding these columns?

I would like to add the columns, like the ones shown in the first picture, to make the arch a little narrower. I just started learning woodworking, and am unsure how to go about adding one. Can you buy those big moldings on top or do you have to build it?

Any advice would be appreciated!
